To refer a patient, fax the provider referral form with the order or clinical indication and any relevant records to 714-441-9454.
Refer Your Client for a Diagnostic Imaging Evaluation
Imaging is performed when clinically indicated following review of the referral and/or appropriate clinical evaluation.
Complete the referral form in the Attorney Referral Packet with your client’s contact details, date of injury and areas of concern.
If you want results and records sent to your office, have your client sign the authorization in the packet. A letter of representation alone is not an authorization.
Fax the forms, your letter of representation if applicable, and any relevant records to 714-441-9454.
Results and records are released to your office only with your client’s signed authorization naming your firm.
